Enrollment

Please call one of our locations to verify that there is availability for your child. Once you have done this, please download the following forms and bring them with you to enroll your child. We recommend that you complete registration process before your child's first day. You can also come to our locations to fill out all the paperwork.

2. Rules and Guidelines
3. Food Program Affidavit
4. Transportation Enrollment Form (if applicable)
5. Infant Feeding Instructions (if applicable)
